Member data matching criteria

To establish you as a Co-op Member, the information on the purchase date of your new insurance policy direct with Co-op, must match the information on your Co-op membership account. It is your responsibility to ensure that these records match and are up to date. We accept no responsibility or liability for the non-application of the offer where inaccurate or incorrect information is provided. The matching process uses the following data:

• your date of birth, and
• your surname and
• your first initial of your first name

You can become a Co-op Member before or on the same day that you buy your new policy. A £1 membership joining fee and Membership terms and conditions apply.

Notification and application of the offer

There is no requirement to enter a promotional code when purchasing a life insurance policy.

Only one welcome gift per eligible policy bought, which will be subject to their own terms and conditions. You will be given access to their website with all the relevant details once you become eligible and are offered the option to choose your gift.

Eligible policies bought in conjunction with other Co-op Insurance member discounts or exclusive offers will still be eligible for this Offer.

How to redeem

If you're eligible, you'll receive a 'How to claim' email within 15 days after your policy has been active for 6 months. It will include details about the offer, how to choose and claim it, and the terms and conditions. We will send it by email or letter, depending on the information you provided when you took out your policy.

You will have 30 days from the date you receive the message to choose which offer you would like. If you haven’t made your choice within the first 15 days, we will send a reminder message by email to let you know you have 15 days left.

If you are eligible for the welcome gift, the message will direct you to a website run by MLP, where you can choose your offer.

After you’ve made your choice, if you choose a gift card, you will see a page with your digital gift card and details on how to use it, including expiry information. If you choose the cheque option, it will be sent to you by post. You should allow up to 15 days for delivery of the cheque from the date of submitting your full name and postal address.

Additional terms

The Offer has no cash value and cannot be exchanged for cash, or an alternative choice.

OFFER FULFILMENT:

If your policy is cancelled within 6 months of the policy start date, for any reason, you will not be eligible to receive the Offer.

The underwriter reserves the right to decline any application for any insurance policy at its absolute discretion and is not obliged to disclose any reason for rejection.
Cancellation or withdrawal

Co-op Insurance reserves the right to withdraw, modify or terminate this Offer, without notice, in whole or in part in the event that it is necessary to do so, or if the uptake of the Offer exceeds expectations.

CHANGES TO THESE TERMS AND CONDITIONS:

We reserve the right to amend these terms and conditions at any time. Revised terms and conditions will be available at coop.co.uk/life-140. It is your responsibility to keep up to date with any such changes.

If any paragraph in these terms and conditions is found by any court or administrative body of competent jurisdiction to be invalid or unenforceable, such invalidity or unenforceability shall not affect the other paragraphs of these terms and conditions, which shall remain in full force and effect.

Relevant law
These terms and conditions shall be governed by and construed in accordance with the laws of England and Wales.
The Courts of England and Wales shall have exclusive jurisdiction in connection with these terms and conditions.

1. eGift Cards may be redeemed in full or part payment for goods in any UK Argos store or online at argos.co.uk, including current promotional offers, at the prevailing prices listed.
2. eGift Cards cannot be used for purchases made over the phone.
3. eGift Cards cannot be used for the purchase of other gift cards or vouchers.
4. eGift Cards cannot be redeemed in ROI.

The risk of loss and title for Gift Cards pass to the purchaser upon our electronic transmission of the Gift Card to the purchaser or designated recipient, or our delivery to the carrier, whichever is applicable. We are not responsible if any Gift Card is lost, stolen, destroyed or used without your permission.
4. Fraud
We will have the right to close customer accounts and take payment from alternative forms of payment if a fraudulently obtained Gift Card is redeemed and/or used to make purchases on Amazon.co.uk or any of its affiliated websites set forth above.
5. Limitation of Liability
WE MAKE NO WARRANTIES, EXPRESS OR IMPLIED, WITH RESPECT TO GIFT CARDS, INCLUDING WITHOUT LIMITATION, ANY EXPRESS OR IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. IN THE EVENT A GIFT CARD IS NON-FUNCTIONAL, YOUR SOLE REMEDY, AND OUR SOLE LIABILITY, WILL BE THE REPLACEMENT OF SUCH GIFT CARD. 6. General Terms
The Amazon.co.uk Conditions of Use & Sale apply. Gift Cards are issued by Amazon EU S.à r.l., a Luxembourg entity. When you purchase, receive or redeem a Gift Card, you agree that the laws of the Grand-Duchy of Luxembourg will govern these Gift Card terms and conditions. If you are a consumer and have your habitual residence in the European Union, you additionally enjoy the protection afforded to you by mandatory provisions of the law of your country of residence. We reserve the right to require additional verification of your identity, Gift Card or account ownership, or provision of an additional payment instrument, before you are able to apply a Gift Card to your account or your Amazon.co.uk Gift Card Balance to a purchase. We reserve the right to change these terms and conditions from time to time in our discretion. All terms and conditions are applicable to the extent permitted by law.
7. Previously Issued Gift Cards
Gift Cards issued on or after July 1, 2013 may be redeemed toward the purchase of eligible products up to ten years from the date of issuance, notwithstanding any stated expiration date. Any Gift Cards issued prior to July 1, 2013 expire in accordance with their stated terms.

If you choose the £140 cheque, this will be sent in the post to the address you supply when you choose this option. You’ll typically receive your how to claim email within 15 days after you’ve made each of your 6 monthly payments. Claimants should allow up to 15 days for delivery of the Cheque from the date of submitting their full name and postal address.

The cheque will be made out in the policy holder’s name. As the policy holder, you will need to have a bank account open in your own name, to be able to bank the cheque. Once the cheque is chosen as the gift, the customer will need to provide MLP with their full name linked to their bank account and postal address via a data capture form. The cheque will be valid for 6 months from the date of issue and should be cashed within this period of time. After this time has passed, you will not be able to cash your cheque.

If you cannot bank a cheque in your own name, you should not choose this option.
